Chester County MH/IDD seeks an Intellectual Disability Supports Coordinator to facilitate, coordinate and monitor services for individuals with intellectual disabilities. Supports Coordinators work with consumers and families in their homes and/or in provider agencies. Job responsibilities include frequent scheduling and facilitation of meetings and entering documentation into a statewide database. The incumbent should have excellent organizational, communication and computer skills. This position requires travel to attend meetings off-site.
Current Child Abuse, State Police and FBI clearances are required, as is a writing skills assessment and an online assessment.
Candidate must have a bachelor's degree which includes or is supplemented by at least 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social science;
or
Two years experience as a County Social Service Aide 3 and two years of college level course work, which includes at least 12 college credits in sociology, social welfare, psychology, gerontology and criminal justice or other related social service;
or
Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and one year of experience as a County Social Serivces Aide 3 or similar position performing paraprofessional case management functions.

Essential Duties, Tasks and Accountabilities:
The position provides case management services to a group of clients with intellectual disabilities living in various community living arrangement programs or their own homes throughout the county. This includes initial intake interviews, appropriate referrals, liability information and completion of progress notes and client contacts.
Minimum Requirements:
The ID Support Coordinator (County Caseworker) position comes under the provision of the Child Protective Services Law. If you are a final candidate for this position, you will have to provide reports on your background from such sources as the Pennsylvania State Police, Federal Criminal History, and the PA Child Abuse History Clearance. If your background is unacceptable, you will be disqualified for employment in this position. You will receive information about these requirements and how to obtain the required reports at the time you are being considered for employment.
Relevant background checks, clearances, physicals, drug tests, and any other assessments or evaluations will be conducted after an offer of employment is given per County policy.
