OB SUMMARY:

The Certified Nursing Assistant (CNA) is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse and performs various services for a patient as necessary to meet the patient's personal needs. The CNA is responsible for observing the patient, reporting these observations and documenting observations and care

performed. The CNA will be assigned in a manner that promotes quality, continuity and safety of a patient's care.

ESSENTIAL RESPONSIBILITIES AND DUTIES INCLUDE BUT ARE NOT LIMITED TO THE FOLLOWING:

  • Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately.
  • Meeting safety needs of patient and using equipment safely and properly (foot stools, side rails, etc.)
  • Giving personal care including baths, back rubs, oral hygiene, shampoos and changing bed linen as often as assigned.
  • Assisting in dressing and undressing patients as assigned. Planning and preparing nutritious meals as assigned.
  • Assisting in feeding the patient as assigned.
  • Taking and recording vitals when ordered with appropriate completed/demonstrated skills competency.
  • Providing proper care and observation of patient's skin to prevent breakdown of tissue over bony prominence.
  • Reporting on patient's condition and significant changes to the Case Manager. Also, aware of the caregiver or other individuals living with the patient and interpersonal issues.
  • Assisting in ambulation and exercise as instructed by the nurse or therapist Support mission, values and goals of Signature Hospice, Home Health & Home Care
  • Maintain confidentiality in accordance with 42 CFR Part 2 and all other established policies, procedures and standards of care Perform other related duties and special projects as assigned

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)

Current, unencumbered, active license to practice as a Certified Nursing Assistant in the state where services are rendered; must maintain applicable state licensure requirements at hire, and for the entire duration of employment.

  • Minimum of two (2) years of experience required; preferably with one (1) year in a home health treatment setting 
  • Current and nationally accredited CPR certification.
  • Knowledge of computer and electronic medical records systems (EMR).
  • Enthusiasm, passion for working with people, and an internal drive to improve the lives of individuals in our care.
  • Utilize de-escalation skills as needed, to maintain personal safety while also ensuring safety for the patients, and other individuals in the home. 
  • Utilize therapeutic communication skills and maintain professional boundaries with patients and their families.
  • Must understand and adhere to the ethical standards of the respective licensure governing board.
  • Must have a valid driver’s license, current automobile insurance, and a safe driving record.
  • Must consistently demonstrate autonomy, organization, efficient time management, confidence, flexibility and cooperation in performing job responsibilities.

  • Stress ranges from low to high, depending on the situation.
  • Job performed in the patient’s home.
  • Requires heavy use of electronic equipment for documentation purposes. Indoor patient care environment.
  • Must have the reasonable ability to stand, walk, sit, use hands to finger, handle, grasp, feel, reach, climb or balance, stoop, kneel, crouch or crawl, talk, see, hear, and drive. Must be reasonably capable of lifting twenty-five (25) pounds. Must be able to safely operate a vehicle; frequent travel within the service area is required.

WAGE: $21.00/Hourly
