LOCATION:
Main office in Randolph. Offsite offices in Dorchester and Medford, Massachusetts. Direct Care staff, based on the program they work in, are designated to regions. Travel for work at patient homes, school and within the community would be required.
JOB SUMMARY:
The In-Home Clinician is responsible for providing community based services; working with both parent/ caregiver and children/ adolescents as a therapist to deliver services in the home and community working as a team with the In-Home Therapeutic Community Case Manager (paraprofessional) and overseeing patient’s treatment plan which direct services interventions. The In-Home Clinician also oversees the In-Home Therapeutic Community Case Manager paraprofessional, ongoing assessment, and integrates evidence based best practices methods and Dialectic Behavioral Therapy (DBT) Skills to stabilize family and care coordination and collaborate with other providers, deliver conflict resolution, using culturally linguistically competent patient-centered strength-based practices. Conducts risk management and safety planning. This also includes coordination with School providers and State Agency Staff with patients consent - staff from the Department of Children and Families, Department of Developmental Services, Department of Mental Health, Department of Public Health, Department of Transitional Assistance, Department of Youth Services, the Commission for the Blind, Commission for the Deaf and Hard of Hearing and the Massachusetts Rehabilitation Commission. Cases are assigned within the Randolph, Dorchester and Medford areas and traveling to patients’ homes is required daily as part of duties.
QUALIFICATIONS REQUIRED AND EXPERIENCE:
This position requires a Masters degree in a Mental Health related field. Must have over two years experience with adults, children, and families or with mentally ill populations, urban youth and crisis management, including training in family therapy, children play therapy Applied Behavior Analysis (ABA) or Substance abuse and Dialectical behavior therapy (DBT).
Must be licensed or license eligible and preferred clinicians have the following minimum qualifications & education:
Psychologists: Doctoral degree in psychology (Ph.D., PsyD, EdD) from an accredited college or university and certification as a Health Service Provider (HSP) by the Board of Registration of Psychology in the Commonwealth of Massachusetts, North Carolina Psychology Board or Florida Board of Psychology
Licensed Independent Clinical Social Workers (LICSW or LCSW): Master’s degree or higher in social work from an accredited college or university
Licensed Mental Health Counselors (LMHC): Master’s degree in the mental health field (including, but not restricted to, counseling, family therapy, psychology, etc.) from an accredited college or university
Licensed Marriage and Family Therapists (LMFT): Master’s degree in the mental health field (including, but not restricted to, counseling, family therapy, psychology, etc.) from an accredited college or university
Candidates must be currently licensed or license eligible in the State of Massachusetts.
Must be Culturally and Linguistically competent, bilingual a plus; Haitian Creole, Portuguese Creole, Spanish, Cantonese, or Vietnamese.
POSITION RESPONSIBILITIES AND TASKS:
Completes intake assessment, treatment plans, CANS medical necessity admission criteria, and enrollment packets for the patient to be enrolled into program services
Must understand medical necessity and be able to diagnose and assess patients participating in program services
Must be able to utilize comprehensive, home-based behavioral health assessment inclusive of the CANS, with fidelity regarding process notes, treatment plans and assessments and discharge plan and progress note in accordance to a policy that meets the professional standards and timeframe and interdisciplinary and/or care review meetings for a plan of care
Completes all documentation needed and develops clearly defined interventions and measurable goals and outcomes that are consistent by a policy that meets the professional standards,
Conducts home visits to enroll patients into the program and for quality assurance compliance as needed.
Collaboration with care coordinator via collard contact (via phone, attendance at IEP meetings, hospital discharge meetings, Care Planning Team (CPT) meetings for youth enrolled in ICC services)
Refers patients when needed to other programs Services for Psychological and Neuropsychological Testing, Medication Management, In-Home Therapy Program (IHT)/ Family Stabilization Team ( FST), Community Support Program (CSP), Dialectical Behavioral Therapy (DBT) Applied Behavioral Analysis (ABA).
Creativity and willingness to engage clients in treatment using non-traditional methods (understand the collaborative treatment of traumatized children and teens, multisystemic therapy, or wrap around services).
Monitors patient’s progress on the implementation of the goals of the treatment plan developed by the patient and family, clinical team and supervisor.
Provides direct client care in 1:1 utilizing a combination of intensive teaching and natural environment training arrangements.
Identifies community resources and development of natural support options for patient and parent/guardian/caregiver.
Direct time with providers (e.g., attendance at IEP meetings, hospital discharge meetings, Care Planning Team (CPT) meetings
Provides referrals and linkage to appropriate supports and services.
Provides coaching in support of decision-making in both crisis and non-crisis situations.
Connect to natural supports and build skills and work on caregiver goals for patient and parent/guardian/caregiver.
Ability to provide behavioral plan intervention and emotional response via telehealth
Coaching in support of decision-making in both crisis and non-crisis situations
Will be trained in DBT (Dialectical Behavior Therapy) treatment methods
Will provide individual case load of billable hours weekly 32 or more
Progress Notes Match Productivity Reports submitted on the 1st & 15th monthly as time sheet for billable hours
Completes Specialty Programs documentation progress notes within 24 hours and uploads to patient file cabinet in EMR
Verifies patients after appointment or services in EMR

SALARY & BENEFITS:
$28 - $47 Fee For Service per hour session with clients per hour depending on licensure and experience
