TITLE: Transition Job Coach / TSW Paraprofessional
SALARY: Classified Salary Schedule - 12 month
QUALIFICATIONS: - Interpersonal skills, respect for confidentiality, demonstrate a positive attitude
- Ability to multitask, prioritize and work in an organized manner
- High level of written and oral communications skills including telephone etiquette
- Have the aptitude of work effectively and maintain a leadership role with students, families, staff, and outer agencies
- Be capable of working a flexed schedule
- Demonstrate ability to maintain accurate and detailed records
- Understand and provide supplemental assistance to students with disabilities
EDUCATION:
• Must be a highly qualified paraprofessional by completing one of the following:
-Possess High School Diploma
REPORTS TO: TSW/Transition Coordinator and Supervisor
PERFORMANCE RESPONSIBILITIES: - Ability to read, interpret, and understand psychological evaluations, IEPs, and other related documents
- Assist special education students to develop and improve life skills for the world of work, such as verbal and written communication skills, electronic communication skills, interpersonal communication skills, and job seeking skills such as applications and resumes.
- Prepare for and attend all VR eligibility meetings for students
- Assist students with researching appropriate post secondary options
- Prepare for and attend all Individual Plan for Employment meetings for students (assist in the development of the IPEs)
- Follow-up and provide updates on students and program weekly with State and Navajo VR
- Set up internships and local businesses for students to work in a field that matches their post secondary goals, strengths, and interests
- Complete all paperwork with students, businesses, and County/Navajo WIA for internships
- Assist County/Navajo WIA with monitoring and follow-up services on students.
- Explain program one on one to students, parents, and families
- Assist each student with all paperwork (applications, release forms, General Medical Examinations (G.M.E.), Individualized Education Plan (IEP)/Evaluation) required for State and Navajo V.R.
- Transport students to and from meetings and conferences in Page and out of town
- Establish appropriate support services at College/Trade Schools, such as coordination with the Disability Coordinator
- Assist students with Financial Aide paperwork (FAFSA, etc.)
- Assist students with housing selections/relocation
- Provide medical transports (Doctor/Counseling appointments)
- Obtain periodic grade and progress checks
- Complete home visits, if necessary
- Assist students in applying for Government Assistance
- Refer to other agencies for further support services (Child Protective Services, Community Behavioral Health Services, Navajo Behavioral Health Services, etc.) if applicable
- Work with businesses to find job opportunities for students
- Job coach by working with students to learn and improve employment skills
EVALUATION:
• Annually by the supervising teacher and/or Educational Services Director
TERMS OF EMPLOYMENT: 12 month
